Occupational Therapy

Our occupational therapists utilize a client-centered approach to strategically develop a custom plan of care that encompasses meaningful and achievable goal outcomes relevant to each patient’s situation. Additionally, each care plan incorporates all involved caregivers and loved ones who oversee patient care to ensure optimal carryover, safety, and well-being of the individual. Our team also evaluates all patient barriers (i.e., co-morbidities, lack of caregiver support, environmental restrictions, etc.) to ensure goal sustainability and patient quality of life. Physical, cognitive, and socio-emotional assessments are an integral part of each care plan to optimize patient function in the home and community setting.

One of the unique features of our teams is our ability to service many different organizations. Specifically, our veteran-owned organization highly values assisting active and retired veterans who serve our country. Additionally, we prioritize and assist many intellectually disabled populations, including but not limited to CTF and Trinity Services of Illinois.
